Care Coordination
At Piece by Piece, we recognize that effective care often requires collaboration across multiple providers and settings. Our care coordination services are designed to ensure that evaluation findings and treatment recommendations are communicated clearly and implemented consistently across the systems that support our clients.
What care coordination includes
With client and family permission, care coordination at Piece by Piece may involve:
- Sharing evaluation results and reports with referring physicians, psychiatrists, therapists, and other treating providers
- Consulting with schools to support the development or revision of IEP and 504 plans
- Attending school meetings, including IEP and 504 team meetings, in person or virtually
- Communicating with teachers and school psychologists to translate evaluation findings into actionable educational supports
- Coordinating with outside specialists including occupational therapists, speech-language pathologists, and behavioral health providers
- Providing letters of support for accommodations in academic, workplace, or testing settings
School collaboration
Our neuropsychologists are uniquely positioned to bridge the gap between clinical evaluation findings and school-based supports. We work directly with school teams to ensure that recommendations are understood, implemented appropriately, and adjusted over time as the student’s needs evolve. This may include guidance on appropriate accommodations, modifications, and related services under IDEA or Section 504.
